How to sign over a title to salvage your car in PA (Pennsylvania)?

I have a tow truck coming tomorrow to pay me to salvage my car. According to the salvage cars section in http://www.dmv.state.pa.us/pdotforms/fact_sheets/fs-buysell.pdf all I need to do is "If you sell your car for salvage, sign the title over to the buyer and remove your license plate. Return the plate to the Department". I have the title in front of me, but there are sections A,B,C,D. I dont know what one(s) to fill out or where I need to sign, to sign the car over for salvage. Please Help.
